Joining a Group or Scheduling a Session?
Please download all documents and fill them out completely.
If you aren’t sure whether or not you need to complete the Information Release, instructions will be given in your first session. If you can’t access these documents through the website, please call the toll free number and they can be faxed or sent by mail.

Fax, mail, or scan and email those documents to Legend Counseling. A portion of your first session will be spent filling out these documents if they are not received ahead of the appointment date. If you are in the Colorado Springs area, you can simply bring the completed documents to your first session.

Counseling Session Payment Information
Sessions can be conducted in person or by phone.
The fee structure is the same for both. Intensives are not offered by phone, so plan to travel if you don’t live in the area. If you do live in the Colorado Springs area, it is advised that at least your first session be in person. Phone sessions are for those individuals and couples who live out of the area. Phone clients can only pay by credit card.
